With guidance from the Director of Population Health Management, the RN Transition Coach works collaboratively with physicians, inpatient case management, nursing staff and the outpatient Population Health Management team to identify and coordinate discharge and transition care integration for patients within the clinically integrated network (CIN), Arkansas Health Network. Based in the hospital, the RN Transition Coach functions as a facilitator of care continuity across care settings, coaching the patient and caregiver to play an active and informed role in care plan execution. With the responsibility of reducing hospital readmissions, the RN Transition Coach is an integral member of the healthcare team who provides guidance to the patient/caregiver for effective care transitions, improved self-management skills and enhanced patient-provider communication. Collaborates with the Arkansas Health Network Population Health Management team on system-wide quality improvement/performance initiatives.','!*!MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Education: Associate Degree in Nursing with RN license plus 5 years relevant experience required; Bachelor of Science in Nursing or Master of Science in Nursing preferred
Licensure/Certification: A current unencumbered RN license to practice in the State of Arkansas is required. Certification as Healthcare Coach or obtained within two years of hire. Certification in case management (CCM), public health and/or community health preferred.
Experience: Five years of clinical and case management, health coaching, or nursing education experience required. Hospital case management/discharge planning experience required. Experience working with Medicare and Medicaid populations and related social/economic issues and/or home care experience preferred.
Skills: Must have strong organizational (time management) skills, strong interpersonal skills
and the ability to handle multiple priorities. Knowledge of and practical use of good business English, spelling, arithmetic, practices and the ability to communicate effectively using written and verbal skills. Proficient in email communications and internet usage along with basic use of Microsoft Excel and Word. Knowledge of information technology to evaluate care effectiveness (care process, outcomes and cost). Basic Life Support (BLS) for the Healthcare Provider certified or obtained by the end of the orientation period (approximately six (6) weeks). Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire. Ability to work autonomously within matrix environment without direct supervision or support.
