ESSENTIAL FUNCTIONS:
Provides pharmaceutical care to patients
assigned by actively monitoring drug therapy, providing interventions and
performing consults for all assigned patients.Promotes professionalism and strives to raise
the level of professional practice.Responds to drug information questions.Provides medication teaching to patients and
families as appropriate.Documents in the patient’s medical record all
pertinent information, including but not limited to progress notes,
consultations.Provides pharmacokinetic dosing and
recommendations as appropriate.Monitor parenteral nutrition and provide
recommendations as appropriate. Participates in on-call responsibilities as
required. Attends in-services, grand rounds, and
teleconferences as directed by preceptor.Prepares and presents case presentations as
required.Prepare and present lectures at Drake
University as assigned.Provide educational in-services to pharmacy and
other hospital staff members and to community health care professionals
and consumers as directed.Actively monitors drug therapy, provides
interventions and consults for all assigned patients.Prepares timely documentation for record
keeping.Participate in quality management activities. Enter, review, and/or
verify medication orders in the pharmacy computer system.Checks medications, including sterile products,
checked by a technician.Participate in interdisciplinary team meetings
or rounds in area of responsibility.Participate as a member of
the Infection Control CommitteeAssist in leading and in the
continued evolution of antibiotic stewardship activitiesPerform day to day
patient-specific reviews of antibiotic therapies and intervene as needed
and as appropriate with prescribersParticipate in Pharmacy and
Therapeutics Committee activities related to infection control, infectious
disease, and antibiotic use. This
includes developing and gathering data related to MUE’s, performing
formulary reviews of ID-related medications, and evaluating and providing
input to the response to adverse drug reactions resulting from ID-related
medication therapies.Assist in directing pharmacy
student activities focused on antibiotic use and infectious
disease-related medications.Provide assistance related
to hospital-wide immunization programs (example: influenza and pneumococcal vaccination
programsComplies with all state and federal pharmacy
laws and regulations.Completes hospital and department orientation.
MARGINAL FUNCTIONS:
Assists
in all aspects of resident recruitment efforts including the interview process.Completes
evaluations of faculty, attendings, and rotations as required.Participates
in committees as assigned.Participates
in Resident’s Forum. Submit
articles for the pharmacy and hospital newsletters as appropriate.Provide
input into the development of policy and procedures within the department or
institution.
','!*!MINIMUM KNOWLEDGE, SKILLS AND
ABILITIES REQUIRED:
Pharmacy
degree (BS or Pharm.D.) from an ACPE approved College of Pharmacy.Completion of ASHP accredited PGY1 Pharmacy Practice ResidencyLicensed,
or eligible for licensure in the State of Iowa.
