Essential Functions
Coordinate and maintain the certification of the Primary Stoke Center under the leadership of Administration and the Stroke Medical Director. Coordinate the development of a Comprehensive Stroke Center for the System. Once achieved follow appropriate guidelines/recommendations to maintain certification.
Identify and follow patients with stroke during hospitalization for the purpose of applying evidence-based practice to improve patient outcomes, both physiological and financial.
Mentor staff in using/fostering the use of evidence-based tools to improve the care of a stroke patient (stroke order sets, nursing care resource guide, patient educational tools). Facilitate neurological assessment skills for caregivers across the spectrum of stroke care. Maintain hospital-wide/system-wide communication regarding Stroke Program.
Develop and facilitate ongoing community and professional education programs. Assist with in-patient stroke education.
Participate actively in the continued development of improved care practices for patients, including practice guidelines and protocols through the use of evidence-based practice.
Demonstrate motivation for continued growth as an educator, and by active review of current literature, professional networking, and attendance at relevant seminars/conferences. Communicate educational offerings to Stroke Team and keep track of the team’s stroke educational hours.
Consults with OT/PT/Dieticians/Case Management/Social Work and other members of the health care team, to continue to identify new strategies for excellence.
Review stroke related data, disseminate with appropriate team members, and establish performance improvement plan to make changes.
Hours: Monday-Friday, 8:00am-4:30pm, may have exceptions for community/staff education events','!*!Qualifications
Required: BSN or equivalent. Masters preferred
Clinical experience with stroke patients. Experience in teaching and/or clinical program development and ordinations desirable.
Minimum 5 years in nursing practice with 2 years in critical care.
Experience with data collection and statistical analysis preferred.
