The Education Specialist – Diabetic Education is responsible for coordinating the delivery of services to patients referred to the Polk Education Center for diabetes self-management training (DSMT), medical nutrition therapy MNT, applying and integrating scientific principles of food, nutrition, DSMT, biochemistry, physiology, management, and behavioral and social sciences under different health, social, cultural, physical, psychological, and economic conditions to the proper nourishment, care, and education of all diabetic population, age-specific patients. Demonstrates professional judgment and knowledge for enhancing quality nutritional care and diabetes self-management to include: assessing the nutritional status and diabetes self-care behaviors, developing nutritional care plans, and/or diabetes management plans, developing individualized programs, and providing education and guidance to staff and key leaders. Services include administrative functions including maintaining recognized diabetes education provider status with the American Diabetes Association. Plan and implement curriculum, individual and group programs.  Evaluate the effectiveness of education by behavior change and demonstrated activities.  Collaborates with patients, nursing personnel and other members of the health care team as needed, including Food and Nutrition Services Department. Ensures patient safety through attention to detail, equipment safety checks, adherence to organizational and department policies, and regulatory guidelines. Collaborates and communicates effectively with leaders, other departments, physicians, peers and patients and cultivates positive working relationships. Displays conduct in support of CHI St. Luke’s Health Memorial Mission & Vision.

Updates and revises patient educational materials as assigned.
Maintains current knowledge of diabetic patient population principles and performance improvement, by reading professional journals and attending continuing education programs.
Conducts quality assurance monitors as assigned and informs peers of recommendations for improved quality of care.
Demonstrates expertise in the teaching /learning process and assists management and clinical staff to develop and provide educational programs.
Identifies learning needs through the use of the Performance Based Development System and Assists the nurse managers, clinical nurse specialists, clinical educators, quality coordinators, and preceptors in planning learning activities to meet those needs.
Develops and performs needs assessments in collaboration with management and clinical staff.
Communicates with staff to facilitate development of their clinical, management, and/ or research skills.
Assesses critical thinking, interpersonal relations, and technical skills of staff to assist in categorization of orientation and staff development.
Consults with management and clinical leaders to develop, revise, and implement cost-effective patient outcomes driven educational programs, tools, and materials for all nursing staff.
Participates in the Performance Enhancement program and other quality enhancement activities through monitoring and evaluating the outcomes of educational offerings.
Collaborates with leadership in research findings into clinical practice, educational programs, and recommendations for policy updates.
Supports and participates in unit based research projects to promote a knowledge base specific to patient needs.
Participates in quality of care activities, collecting quality data, analyzes the data, and formulates action plans to improve quality care.
Exhibits specific leadership skills, including: delegation and follow-through to non-licensed staff, positive interpersonal skills, and effective and efficient coordination of all aspects of patient care.
Demonstrates commitment to personal growth and development, and professional practice.
Maintains applicable licensure and certifications, completes annual mandatory in-services and code validation as required.
Uses interventions substantiated by research as appropriate in the patient population, participates in research activities, identifies opportunities for research, and collaborates with departmental & nursing leaders.
Supports department-based goals and contributes to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
Communicates with patient's family, physicians, and peers in an accurate, courteous and professional manner.
Utilizes the nursing process to ensure appropriate patient care is provided in a cost effective, quality driven, and patient focused manner.
Performs duties in accordance with that of CHI St. Luke’s Health Memorial policies and procedures, and regulatory guidelines.
Performs other related duties as requested or assigned.','!*!Minimum Qualifications:




Education Level

Minimum:
Graduate of an accredited nursing vocational program.
Preferred:
Bachelor’s Degree bachelor’s or graduate degree from a regionally accredited college or university in one of the following majors: Human Nutrition, Food and Nutrition, Nutrition Education, Dietetics or Food Systems Management.


Licensure/
Certification

Minimum:
Licensed in the state of Texas as a Vocational Nurse (LVN)
BLS certification (through an approved AHA program)
Preferred:
Licensed in the state of Texas as a Registered Nurse (RN)
Certified Diabetes Educator (CDE)

Experience

Required:
Three (3) years clinical education experience
Preferred:
