Essential Job Duties:
The primary purpose of this position includes but is not limited to:
This position will join a multi-disciplinary team (psychiatry, nursing, social work, occupational therapy, etc) providing community-based services to adults living with severe, persistent mental illness. This position works closely with the Psychiatrist and Team Lead, as well as primary care physicians, pharmacy and others to ensure integration of behavioral and physical healthcare. Responsibilities of this position include administering long-acting injectable medications, coordinating physical healthcare for clients, oversight of the team's medication management system, providing education and health promotion interventions around healthy lifestyle choices, and other tasks essential for the client's services. The services are provided primarily in the field (home visits, attending medical appointments, etc).

Required Qualifications:
1) Bachelor degree/diploma from an accredited School of Nursing.
2) Currently licensed RN in North Carolina.
3) Valid NC Drivers License
Preferred Qualifications:
Experience in providing offsite healthcare.
Required license or certification:
Current RN License for the State of North Carolina
