What You Will Do:

Main Function
Under general supervision, leads in the overall design and implementation of the unit's education program and facilitates achievement of the highest quality patient care, as measured by positive clinical patient outcomes, and enhanced customer satisfaction. Leads unit governance activities, and leads or actively participates in organizational professional governance initiatives. Mentors others to achieve and grow in their profession. Monitors the quality and safety of the education program, leading unit based audits and continuous quality improvement projects.
Duties and Responsibilities
The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.
Educates and counsels patients with diabetes and their families, assessing their physical, psychosocial, and economic needs, coordinating their care across disciplines, and promoting positive self-management skills.
Leads the patient education program on the unit, including individual counseling and group education, the development of appropriate educational materials, the development of new programs, and the assessment of new diabetes-related products, clinical practice protocols, staff schedules and in-patient support. Evaluates program effectiveness using measurable outcome audits, and patient satisfaction data.
Serves as coordinator for educational communications with affiliate program managers and oversight of the continuous quality improvement throughout the network. Ensures the meeting of American Diabetes Association Education Recognition Program (ADA-ERP) standards and oversees the renewal and maintenance of ADA-ERP.
Researches and integrates evidence based clinical findings into practices on the unit.
Coordinates community outreach including formal and informal requests for information and/or assistance regarding diabetes and diabetes-related health care issues, health fair activities, and speaking engagements.
Oversees the development and use of the educational components of the electronic medical record, serving as advisor to IT development team and educators, and conducting audits to ensure regulatory compliance.
Keeps current on the latest in diabetes care, educational techniques and products available to maintain a state of the art knowledge base and serve as clinical expert and resource to others.
Mentors and coaches other educators and support staff encouraging and guiding their professional growth and enrichment. Teaches others formally and informally on evidence based practice applications.
Oversees the organizational and community targeted professional development activities, including the annual professional symposium, and Diabetes Management for the Bedside Nurse.
Oversees annual competencies for the diabetes educators; mentors and coaches educators to achieve and grow in their professional role.
Leads efforts for patient/family satisfaction and service recovery in the unit.
Leads and participates in performance improvement and continuous quality improvement projects by designing, implementing, evaluating and disseminating results.
Trends quality data such as Midas, NCQA, and educator chart audits; leads efforts to address deficiencies.
Assists in preparation of unit and staff for the Joint Commission (mock and actual) surveys and other regulatory site visits.
Actively participates in the selection of new staff members. Develops and oversees the orientation and evaluation of new staff members. Coordinates in-service education as needed for staff development.
Leads unit governance initiatives, and leads or actively participates in other Ambulatory Service division and UMMC organizational governance initiatives.
Oversees the unit patient/family support group.
Actively participates in the unit's annual Advisory Meeting and serves as liaison between the advisory group and the Center's administrator.
Serves as spokesperson for the unit within and beyond the walls of the institution; works closely with the marketing department to develop appropriate print and multi-media promotional materials .
Promotes referral development for education in the unit by acting as liaison to primary care and specialist physicians. Serves as manager in manager's absence.
Knowledge, Skills and Abilities
Highly effective verbal and written communication skills are necessary in order to educate patients and their families and work with medical, nursing and management staff. Must have demonstrated ability in public speaking, and the ability to learn and use computer systems.
Must demonstrate ability to lead others, such as effectiveness as leader of unit/organizational initiatives, charge nurse, mentor, or clinical resource. Demonstrated effectiveness as a multidisciplinary team member
Must maintain active membership in a relevant professional organization.
Maintenance of 15 contact hours of education per year.
Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.
Patient Safety
Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives.
Takes action to correct observed risks to patient safety
Reports adverse events and near misses to appropriate management authority.
Identifies possible risks in processes, procedures, devices and communicates the same to those in charge.

Working Conditions
Weekend, shift work, holiday, on-call, and overtime may be required.
Work is primarily performed in a standard office environment, but also requires contact and work with staff in a clinical setting.
What You Need to Be Successful:
Education and Experience
Bachelor's degree in nursing, nutrition, dietetics, or other healthcare related discipline is required. Master's degree preferred.
Must hold licensure as a dietitian or nurse in the state of Maryland, or eligible to practice due to Compact state agreements as outlined through the Maryland Board of Dietetics/Nursing.
Must be a Certified Diabetes Educator (CDE), or Board Certified-Advanced Diabetes Management (BC-ADM).
Four years experience as a Diabetic Educator, preferably in a community health or ambulatory care setting.
Active CPR certification is required.
Maintains membership in a professional healthcare organization (nutrition, nursing, diabetes or a related field).
