Diabetes Educator Role:
Under general supervision, assesses, plans, implements, and evaluates several components of the diabetes education program at the diabetes and endocrine practice. Also serves as the diabetes nutritionists for patient education. Plans and conducts continuing education programs in diabetes and nutrition for allied professionals and the general public.
Duties and Responsibilities:
1. Works with the Medical Director and Regional Practice Manager to facilitate and maintain Diabetes Provider Recognition for the endocrinologists in the program. 2. Manages all aspects of criteria required for NCQA accreditation with assistance from the Regional Practice Manager as needed. 3. Promotes referral development for education in the Center by acting as a liaison to primary care and specialist physicians, participation in health fairs, and other marketing/community outreach activities. 4. Plans, assists and implements the development and coordination of educational programs and seminars focusing on the management of diabetes, diabetes-related problems and diabetes education for healthcare professionals and support staff. 5. Participates on grant initiatives as requested. Maintains and provides reports to Administrative personnel as required. 6. Responsible for maintaining the following surveys, audits and reports: a. Quarterly Patient Satisfaction Surveys b. Clinical Outcomes Audits c. Referring Physician Satisfaction Surveys d. Educator Audit e. NCQA statistical data and reports
What You Need to Be Successful:
Bachelor's degree
in healthcare related discipline or equivalent is
required.
Certification as a Diabetic Educator (CDE)
is
preferred.
A minimum of two years experience in diabetes care and education is required
. One year in a community health or ambulatory care setting is preferred.
