Job Summary
Under limited supervision, directly provides oversight and coordination for supporting a robust enterprise-wide diabetes management and education program. This position will collaborate with physician leadership, administration, professional, and other staff throughout the organization, as well as the organization's community health, department of health & social services partners to sustain and grow the diabetes programs and population health department and promote a culture consistent with the health systems mission, core values, and standards of conduct.

What You Need to Be Successful:
Graduate of an accredited school of Nursing and be currently licensed as a Registered Nurse in the State of Maryland; BSN preferred.
Ccurrent certification by the National Certification Board for Diabetes Educators.
Minimum of two years' experience with care of persons with diabetes and diabetes-related illnesses.
Previous management or coordinator experience preferred.
